Notice Regarding Request Form Fees and Payment

Effective June 1, 2010, the Business Division will no longer accept request forms without payment. Previously, Certified Order Forms and UCC 11 Information Request forms could be submitted without payment and the customer would be billed for the amount. Request forms must now be accompanied by payment.

In order to facilitate advance payment, the fee structure for these forms has been simplified and customers will no longer be changed a "per page" fee for copies. See the Fee Schedule for a complete list of fees.
